Job Description

Position Summary: Is responsible for the overall management of the provision of services to each child, foster boarding parent and biological family assigned to his/her caseload.* This includes assessment of needs of each individual and the detailing and implementation of a treatment plan designed to effect the earliest possible discharge of each and every child to a permanent living situation.*

Position Responsibilities:

  • Providing whatever services are indicated to promote the best possible adjustment for the child and to facilitate meeting his needs emotionally, physically, and spirituality.* This includes, but is not limited to:
  • Arranging for psychological and psychiatric assessments when indicated.*Carrying out the recommendations of the above, within prescribed time frames.*Monitoring educational progress and making appropriate referrals for indicated educational programs.*Monitoring spiritual development through child's attending services in religion chosen by parents.*Ensuring, in conjunction with the community service nurses, appropriate medical examinations and treatment.*Monitoring the provision of age appropriate recreational opportunities.*
  • Maintaining the relationship between and child and bio parent by sharing with each significant information about the other and facilitating frequent visiting in accordance with the standards set forth in Adm. 100, Utilization Review and ACS guidelines between bio parent and child.*
  • Maintaining regular contact with the foster parent, at least monthly, in order to facilitate the child's adjustment to the foster home.* This includes sharing pertinent information about the child's past, interpreting problem behaviors, and offering suggestions around more effective parenting techniques.*
  • Continuing interpretation to foster parents of their role and how they can help in promoting the child's adjustment and implementing the permanency goal.* This includes, but is not limited to:

Assessment of the reason for placement.*

  • Obtaining personal and family history in order to appropriately assess both individual and family functioning and identify strengths and needs.*
  • Establishing goals and specified time frames with the biological parent for implementation delineating who is responsible for specified tasks in order to achieve these goals.*
  • Reviewing these goals on a regular basis.*
  • Providing a supportive relationship to help the biological parent to achieve the above.*
  • Referring the biological parent to appropriate services which will aid in the achieving of the above, i.e., welfare, housing, mental health.*
  • Continuing to share with the biological parent pertinent information about the child's adjustment and involving them in decision-making when indicated.*
  • Providing whatever services are necessary to effect the earliest possible reunion of the child with bio parent or another family member, or if this is not possible, to expedite the child's adoption.*
  • Completing the necessary written work to ensure our accountability to City, State, and Federal guidelines. This includes, but is not limited to:

Running record dictation or progress notes summarizing all case activity.*

  • Social worker must complete the following documentation within required time frames:

Family assessment service plan [FASP]*Referrals for any needed service.*Correspondence.*Permanency Hearing Reports.*Psycho-social summaries. Any required reporting form to ACS or NYSDSS.*Case management sheets.*All data entry into CONNECTIONS.*To participate in weekly supervisory conferences with the supervisor and share significant case activity.*

  • Formally review goals and objectives via the Service Plan Review update.*
  • Testify in court.*
  • Attend case conferences.*
  • Participate in In-Service Training.*
  • Attend monthly staff and unit meetings.*
  • Additional duties/responsibilities as required to meet the needs of the department/agency.
  • Performs work accurately and thoroughly on a consistent basis.

Essential skills, experience, licensure, certification:

  • BA, BS, BSW, and a willingness to learn required.
  • Computer literacy.
  • Experience working with children and families is a must.
  • Fluency in Spanish and English language is required.
